ACL Reconstruction
Anterior cruciate ligament (ACL) reconstruction is a surgery involving tissue graft
replacement of the anterior cruciate ligament, located within the knee, to up-turn its
function after an injury. The torn ligament can either be preserved before reconstruction by
an arthroscopic procedure or removed from the knee.
Symptoms
- Rapid
swelling in the injured knee
- Pop
sensation in the knee
- Loss
of stability and range of motion while walking.
-
Severe pain in the injured knee

Process
- 2-4 small incisions
are made around the injured knee
- An arthroscope is
inserted through one of the incisions for inspection of injury.
- Firstly the torn ACL
is removed and the area is cleaned.
- Through other
incisions, small holes are made into femur and tibia with surgical drills.
- Then graft is passed
through these holes and held in place with
- screws/plugs/staples.
Incisions in the skins are then stitched or stapled
Post Procedure
- Post surgery, care is
taken to keep incisions dry & clean to avoid infection
- Patients need to wear
Knee Braces for 2-4 week. Physical rehabilitation is necessary to gain stability in motion.
This could take approximately 4-6 months.
Diagnosis
It's important to understand the severity of the injury and get the best possible diagnosis.
- Clinical
Examination: clinical examination is done to understand injury and functionality of
knee joint is checked by moving it in different direction
- Pivot shift
test: This test helps us in understanding instability in ACL injury
- Anterior drawer
test: The patient is
- supine with his hip
flexed at 45˚, knee flexed 90˚ and feet flat at table. The tibia is then drawn forward
anteriorly. If the tibia pulls forward and backward more than usual, the test is considered
positive. Excessive displacement of tibia anteriorly implies injured ACL.
